Mental Health
- Mental Health services are provided to clients in their place of residence
- The service is provided by a nurse practitioner and can act as the liaison between the primary care MD and/or psychiatrist
- Monitor and manage medications
- Develop community support network and/or recovery based program
- Guidance with dementia and depression and working and communicating with family members
- Assessment of psychosocial issues as impacted by serious and persistent mental illness
- Assessment of risk factors (suicidal, homicidial, etc.)
