Community Home Health was established in 2006 by owners Phil Davidson, Mary Theiler, and Chad Hillyer. As owners of several out patient rehab clinics in Omaha and surrounding communities, Phil and Mary saw the need to add to the services they were already providing and continue the continuity of care offered to clients in their homes.
Our Goal
The mission of Community Home Health is to provide excellent customer service that includes personalized patient care, prevention, and consistent communication with the client, family, medical doctor and staff of the community.
Our Approach
We strive to provide quality care rather than focus on quantity of care. Providing a proactive approach to health care rather than reactive is the hope to prevent illness, injury and hospitalization which in turn reduces health care costs.
How We Work
Community Home Health provides an open communication policy. We strive to provide communication between the client, family, doctor and if staff from a facility is involved. CHH feels this is a crucial part of providing quality care and to get the most out of the client and services provided.
Our staff is trained to go beyond the call of duty. CHH will be sure that the upmost care and communication is given to our customers and everyone involved. Customer service training is incorporated into the orientation of new hires and consistently provided throughout employment.
Consistent Staffing
We work hard to keep the same staff with the same client. We believe this creates better health care, better communication and a level of trust is gained and maintained. With the consistency of staff provided, the client has a higher percent rate of meeting their goals that has been set up in the Plan of Care signed by their doctor. The progress of the client’s status is observed more clearly and is defined as progression is being made.
