What services does Community Home Health offer?

CHH offers skilled nursing, physical therapy, occupational therapy, speech therapy, mental health services, and medical social worker services.

Do you have a bath aide?

We do not utilize bath aides. CHH’s philosophy is to use occupational therapists rather than bath aides. This allows the OTs to provide skilled rehab when bathing and other activities of daily living. The patient will receive education and exercises that will in most cases allow the patient to gain independency with these activities much sooner and much safer.

Does Medicare cover home health?

Medicare covers home health services 100%.

What insurances does Community Home Health take?

CHH takes most insurance companies except Conventry. Most insurance companies will cover 100%.

Does one need a doctor’s order to receive home health?

An order from a medical doctor is required in order to receive home health services. The order cannot be signed by a Physician’s Assistant or a Nurse Practitioner, only by a MD.

Can I go places when I am receiving home health?

It is required that the client is homebound when receiving home health services. This means that the client cannot drive, it is a tasking effort to leave home, requires assistance while not home and is a tasking effort to return home.

How long will I need home health services?

The goal is to provide the skilled care needed and to progress the client to improve function and healing. The treatment provided must be medical necessary and when there is evidence of no change is health status or improved status that the client can be independent and safe, then home health services will be discontinued. The entire plan of care is approved and consistently updated with the physician involved.
